316 Evidence-Based Pain Management Practices for Older Adults in Multiple Practice Settings: The Challenge of Translation

Herr will moderate and provide an overview of the problem of under-treated pain in older adults and the challenges of consistent use of evidence-based pain management practices. Ersek will provide data documenting adherence to evidence-based practice use in long-term care based on a sample of twelve nursing homes and over 250 patients with chronic pain. Fine will address evidence-based practices used in hospices providing community-based care for older adults with cancer pain based on data from sixteen hospices and over 350 patients. Titler will conclude with an overview of a model of translation interventions to facilitate adoption of evidence-based practices and will discuss sustainability of practice change in an acute care setting based on data from twelve hospitals and 2459 patients.
